Instructions: This application must be completed by the parent or legal guardian of the student requesting enrollment under the State of Michigan Schools of Choice Program. Section 105 applies to students residing within an intermediate school district (ISD) contiguous to Livingston ESA. Section 105c applies to students residing in an ISD non-contiguous to Livingston ESA. All requested information and required attachments must be provided for the application to be processed.  Fowlerville Community Schools will accept students from the following counties Genesee, Ingham, Oakland, Shiawassee, and Washtenaw.

Applications will be received until Monday, August 24, 2026.  

Students must be enrolled and attending Fowlerville Community Schools by Friday, August 28, 2026.

Please answer accurately. Failure to disclose disciplinary records may result in the immediate denial of the application or subsequent exclusion from Fowlerville Community Schools.
Has this student been suspended (in-school or out-of-school) within the last two (2) school years?   (required)
Does the student currently receive Special Education services or have an active Individualized Education Program (IEP) or 504 Plan?   (required)
If yes, a copy of the current plan must be attached below to coordinate services with the resident district.

Note regarding transportation: Parents/guardians are responsible for providing transportation for all Schools of Choice students to and from Fowlerville Community Schools.
